  Welcome to the Event Registration Form for Student Activities. This must be filled out for any club or organization that is recognized by Student Government Association prior to planning a club event. After the Office of Student Activities has reviewed the event proposal, an e-mail will be sent to the club e-mail indicating whether or not the event has been approved.

THIS DOES NOT MEAN THAT YOUR ROOM IS RESERVED! You will also need to submit a facilities request form to the appropriate department for approval.

All areas on campus require to be reserved via the facilities request form. If your event is off campus, please answer "N/A."

Note that when money is collected, cash boxes and receipt books are available at the Cashiers office, any money collected will need to be deposited by the next business day into your fundraising index at the Cashiers office with a deposit slip.

Please note that the Travel Authorization Form for all trips are due a minimum of two weeks in advance.

If no money is needed, please answer "None Needed." Please note that all vouchers are due at least 5 business days in advance of the event. Paperwork to hire individuals are due at least 2 - 4 weeks prior to the event. If a contract is needed, make an appointment with Student Activities to prepare contract before sending to service provider.

Please be specific and concise. This description will be used to advertise your event on the Eastern Calendar of Events if approved.
 
   
  Please hit the "Done" button when all of the questions have been answered. An e-mail will sent to the club e-mail approving or denying the event. A follow-up e-mail will be sent once the event has occurred with instructions to fill out an evaluation. If the event is changed or cancelled, e-mail the Student Organization Assistant with those details.
